As a new Blood Bank supervisor I'd like to get input from you on a sticky subject.
How do you let your co-workers know that when you are performing blood bank work at the bench the interruptions need to be kept at a minimum? I'm often interrupted by casual greetings or chit-chat and find this very frustrating.
I realize I could post a big sign that says "QUIET IN THE BLOOD BANK," but I'm hoping for a less offensive way of getting the message accross.
Ideas?

I'm thrilled to have found this forum and have been lurking around for a while gathering a great deal of info.
I've been a generalist CLS for 25 years and, after a sudden departure of our Blood Bank supervisor, have taken on the role. Though I worked in Blood Bank frequently I'm amazed at how little I really knew of the day to day workings of the Blood Bank. I'm working hard to get up to speed but it has certainly been a challenge!

Stem Cell Experience in Ecuador

When the Echo gives you" ?: reactions
Lately we have seen more ?'s than usual on our Echo. Normally they are in the group but lately they have been popping up in the screening. How is everyone handling this? Are you working up each ? in your screen cells? Are you looking at them and reporting them out visually? Are you ignoring them? We are spending lots of time discussing it and were wondering what anyone else is doing?

Other FDA reportable?
Do you think this is FDA reportable? I am leaning toward "yes". A patient had a WAA. It was positive in gel, LISS and at 30" saline. Patient has not been transfused since 2005. Our SOP states an absorption needs to be done to rule out clinically significant antibodies. Absorption was not done. Physician signed form for incompatible blood. Two units were dispensed to the patient.

minimum volume of FFP?
Is there a Standard as to how much minimum volume should be there in a unit of FFP? Also, do most hospitals issue FFP based on ml/kg or Units? Thanks.

Off site clinic transfusions
Our hospital supports three clinics close to us. In one we transfuse out-patients. The staff is employed by our facility, the nurses and doctors are trained in transfusion policies and procedures, and follow all the main hospitals guidelines. For this reason we allow transfusions to be done there.
Is there a regulation that states what the actual rules are for off site transfusions?
A question has come up as to wether it is legal and if we can transfuse there why not at the local cancer center (which is not at all affiliated with us.)

Validation HELP
I am in the process of validating our Hematology analyzers to perform platelet counts on platelet rich plasma from our plateletphersis. Has anyone ever done this? If so, can you share what you did to validate, the statistical methods you used, and your acceptable criteria? The analyzer does not matter. I don't want to reinvent the wheel if I don't have to. Thanks!

Filter size for neonates
Hello all,
I am looking for references related to appropriate filter size for neonatal transfusion of blood components. Most of the items that I am finding reference the adults or filter size of 170-260 microns, yet there are some products out there with different filter sizes.
Any suggestions??

Stickers on Blood Bags
Our nursing staff is thinking of asking us to place a "charge sticker" on each unit to remind them to charge for their supplies and services. I know that they will have to be of a certain quality. Any thoughts?

Can we issue blood products for porters or nurses ?
Good morning/evening for everybody
we have a problem with the maneger of our hospital that it is not the duty of the nurse to pick up the blood from blood bank but we told him that the porters are less educated and may not handle the blood product properly.
Please is thier any policy or recommendations from you or from aabb?

Another Cooler Question
Hi:
A question was raised (again) today on what temperature whole blood units should be stored at the blood drive and during transport to the BB for processing. I am in recruitment and collections so can use the BBers help on this one
My understanding is that we are supposed to "start the cooling process" toward the 1-6c range and there is (and can not be) any specific temperature. If I place blood collected at 37c into the cooler and check it in 30 minutes it may be 30c or 25c. If I constantly place warm blood into the cooler, then it may never reach that 1-10c range.
I think collected blood, pre processed, is different from storage 1-6c or transport of blood 1-10c.

Blood Ordering Policy
How does your hospital handle physician orders for blood? My specific question --if a patient requires special units such as irradiated, CMV neg, Sickle neg--does the physician have a written order specifiying these requirements or is it the responsibility of the Blood Bank to recognize the need and give appropriate units. We have run into some seroius discussions with the orders matching the products. Most of our docs order PC period. It is often the Blood Bank who calls and questions or recommends. Would like your thoughts and input.

Apheresis Platelet Collections - Platelet Counts for Yield Calculation
