Revised MHRA Inspection Manuals
Below is a link to what MHRA inspectors are now getting trained on for inspecting blood establishments, there is also a the manual gives valuable information and guidance on the structure of documents related to quality management systems. The content of this manual will therefore by included in the EU co-funded project on developing common criteria and standards for the inspection of blood establishments.

Age limit for automated donors?
Hello everyone:)
I realize in most states donors have to be at least 17 to donate blood. Does your facility's have an upper age limit for new automated donors? We have the Trima, Amicus and Auto-C. I have looked in standards, AABB Technical manual, Internet, etc. There is a discussion at my facility about this. We are getting donors coming in that are 70 and above expecting to donate pheresis, with a vein that will not handle the pressure. When we suggest they are best for whole blood they are disappointed:frown:, even though we explain they are helping save 3 lives. Any input would be appreciated. :) Thank you.

Partial D
I have an academic question to post. Does anyone know if the low incidence antigens (ex. BARC, FPTT, TAR) that are associated with partial D are located on the Rh protein? Are they neo antigens that are formed as a result of the gene mutations in partial D's ?

Supply of DNA for rare antigens
Does anyone know where you could begin searching to obtain a tube of blood from donors with rare antigens? We would like to start cell cultures to have a supply of rare antigens to QC molecular testing.

Anti- D present with and negative Medical History
A D - lady presently pregnant with history of one previous delivery of a D- baby, developed anti- C post delivery. Now 4 years later we see she has anti-D. The titer of her antibodies was 8, 4 months ago, is now 2.
She has not yet recieved RhIg for this pregnancy.
So what happened ? why the +1 for anti-D (she has +2 for the C)??

Can I charge for a RedCell LeukoReduced P9016 even though just a Packed RedCell P9021 is ordered?
Our blood supplier only sends leukoreduced Packed cells. If we write a reflex policy that informs the doctors that an order for Packed Red Cells will be charged as a Packed Red Cells-Leukoreduced- is that legit?

ABO discrepancy SOP

ABO Mistypes
Recently my facility ahs had an increased number of ABO mistypes. We have had 10 for the year. For the most part the tech that is responsible for the mistype catches it anywhere from 10 minutes to an hour after. They call the floor to notify that there is a change in results. I have been writing an occurrence for this even if they catch it themselves. My question is am I being to strict? If they catch it themselves should I not write the occurence? What about cGMP? What is an acceptable number of ABO errors? Thanks for any imput you can provide. :confused:

platelet swirl
Since lack of platelet swirl is associated with decreased platelet viability, I'm curious how many check for platelet swirl before issuing platelets? If so, how do you document this check and do you discard those which do not demonstrate swirling? We only purchase SDPL's and pre-pooled LR platelets, but some do not demonstrate swirling.

We are a Cerner Millenium system and I just heard about the Bridge product that can be used on Cerner for tranfusion related documentation. Does anyone have experience with this program; if so, how do you feel about it? Is it a good system, what problems have you seen, etc?

My pathologist asked me to see if anyone is using or has any experience with a Belmont Rapid Infuser. Apparently our trauma surgeons approached him about acquiring the device. I have a brochure from the distributor but I cannot find a specific model number or product code or name, just "Rapid Infuser".
I'd be interested in any and all opinions -- good and bad -- about the device.

Unit Type Confirmations on the ECHO
For those of you performing unit type confirmations on the ECHO, are you experiencing alot of probe clot detected errors?

Platelet testing using the verax system
Has anyone tested there platelets using the Verax method to detect bacteria? We have just started it and we are encountering alot of false positives? Any suggestions?

Cytotherm Plasma Thawer
Our facility is looking at buying a new plasma thawer to replace our waterbath. We are trying to find something that will thaw faster and take up less counter space. We are looking at the Cytotherm 4T model that has the capacity of 1-6 units at a time. It will also allow for a second corral that will allow up to 12 units at a time. Does anyone currently have this plasma thawer? Also, what are your thoughts?

CMV test vs. CMV safe
When we have a request for CMV negative, we currently have our blood center send us tested CMV negative units. We give all leuko-reduced products and are seriously thinking of just going with the terminology CMV safe and save money and trouble.

Echo DAT's
We just went live with our Echo this week. I am curious if anyone performs DAT's on their Echo. Most of our DAT's are from cord bloods. Have you had any problems with cord blood? Is it worth the time to perform the DAT on the ECHO vs tube method? Considering that cords have Wharton's jelly does this preclude you from running it on the ECHO?
